Please use the Signups are still available at the link and QR code below or by contacting Monica Burns at brns1@uif.uillinois.edu.
The day of the blood drive, donors should visit the ImpactLife website to complete the medical history questionnaire.
Important Notes:
- This must be completed the day of donation (after 12:01 am CST)
- The donor must retain the QR code generated after the form is completed. It is recommended to take a ‘screenshot’ of the QR code and send it to their email. Unfortunately, without the QR code, the donor will have to complete the medical questionnaire again.
- In the event of no/poor connectivity, the donor may need to retake the question.
